I think if the person you are talking to is indeed reading a book at present, he or she can fully understand that 你在看什么？ refers to reading. No need to say 你在看什么书 (I know you want to ask for content itself instead of the name of the book).
However, you may also use the sentence when the person is watching TV, or looking around seeking for something (but in this case often we will directly say 你在找什么, i.e., what are you looking for). Therefore the meaning depends on the context. If you use this in conversation, I think there is no problem.

"念" has two meanings. The most common meaning is reading aloud (moderate loudness of voice that the person who wants to hear is able to hear). Another meaning exists in the phrase "念书" which simply means study, just like the phrase "读书" has a partial meaning of study as well (we do not often use 念书 for the pure concept of reading however). But if you say "看书", you are not necessarily studying.
